New Patients


Please double check that your home address is within our practice catchment area by either checking our Google Catchment Area Map or by phoning the Patient Advice and Liaison Services at the Greenwich Primary Care Trust on Freephone 0800 1699 928. They are available 9.00am - 5.00pm Monday to Friday.

Registrations take place at:-

Gallions Reach Health Centre – Monday to Friday 11.30am to 2.30pm as a walk in service


Heronsgate Medical Centre – Monday to Friday 4.30pm to 5.30pm by appointment only

If you wish to register at this practice you will need to produce the following current documents:

At registration any person under the age of 16 will need to show their birth certificate showing their parents’ name.

These documents will be copied and retained in your medical file.

Failure to provide documentation may result in the refusal of your registration.

When you have supplied your details, you will be asked to make an appointment with one of the Practice Nurses to have a New Patient Check. We are required by the National Health Service Regulations to carry out this health check on all patients requesting registration.

Your new NHS Medical Card will take between 2 and 3 weeks to arrive from the Health Authority.
