In Greenwich far too many people live with preventable illnesses which reduce their quality of life and can lead them to die too early and what’s more, our most disadvantaged communities in the borough are disproportionately affected. Unhealthy lifestyle behaviours such as smoking, poor diet, drinking too much and lack of physical activity directly impact on both the length and quality of life. However, the underlying wider determinants of health, including poverty, worklessness, poor housing and environments, and social isolation have a huge role to play in the health and wellbeing of our residents and communities.
The continuing high levels of need, limited resources and the diversity of the population in Greenwich requires us to shift our resources towards prevention, make best use of community assets and to work together across organisations with an increased focus on the wider determinants of health.We are currently piloting new Live Well Coaches for patients in this GP Practice for registered patients who are over 18 and needing help and support related to the issues listed below.
The new Live Well Coaches are trained, local people who have knowledge about local services and skills and experience in supporting people to get the help they need with issues like:
