We Are Now Using Total Triage for All Appointment Requests
Thamesmead Medical Associates has now adopted a total triage model for all appointment and clinical queries. This means that all patient requests should be submitted online using our secure form, which is available via our website.
Once a form is submitted, our clinical and administrative team will review the information and direct you to the most appropriate service. This might include texting you back or calling you back with basic advice or :
• A GP or nurse or HCA appointment
• Referral to a community pharmacy under the Pharmacy First scheme (for certain minor illnesses)
• Signposting to another member of our multidisciplinary team
• A self-referral route for services like physio, mental health, or antenatal care
• Administrative help or a prescription request
Why We’re Doing This
Our goal is to ensure that patients with the greatest clinical need are prioritised, and that we make the best use of NHS resources. By triaging all requests, we can improve access, reduce waiting times, and increase convenience for everyone.
Help Us Help You: Be As Detailed As Possible
When filling out the online form, please include as much relevant detail as possible. This really helps us triage your request safely and efficiently.
If you’re requesting a fit note, for example, let us know:
• What the fit note is for
• Whether you want it texted, emailed, or collected at reception
If your form does not contain enough information, we may need to return it to you for clarification, which can delay your care.
Can’t Go Online or Need Urgent Help?
If your query is urgent or you’re unable to go online, you can still call the practice and our reception team will assist you. Our online forms are accessible via our website during core practice hours, and our receptionists can help you fill out the form if needed.
